NDIS Referrals - Allied Health

We provide services under the Improved Health and Well-being section for those with a permanent disability, where that disability impacts on the ability to perform daily activities.

Once an NDIS referral has been received and processed, an NDIS service agreement draft will be drawn up for the participant and their family to sign. Once completed, we will allocate clinician/s and an appointment will be made by contacting the participant or next of kin (if appropriate).

Participants will be accepted who are self-managed, plan managed or NDIA managed. It is highly recommended that the NDIS plan be forwarded with the referral to gain a thorough understanding of the participant’s goals and allocated funding.

About the NDIS

The National Disability Insurance Scheme (NDIS) aims to provide all Australians under the age of 65 years, who have a permanent and significant disability, with the reasonable and necessary supports they need to enjoy an ordinary life. The NDIS helps them achieve their goals.

The NDIS was first rolled out in Victoria in July 2016 and now all eligible Victorians now have access to the NDIS from July 2019.

This may include greater independence, community involvement, employment and improved well-being. As an insurance scheme the NDIS takes a lifetime approach, investing in people with disability early to improve their outcomes later in life.

To become an NDIS participant a person must:

Have a permanent impairment
Be aged less than 65
Live in Australia in an NDIS area
Be an Australian citizen

If you are not sure whether you are eligible to access the NDIS an eligibility checklist can be found here.
