Exercise Physiology Medicare CCM / EPC / CDM 2025/26
There are several changes to Medicare Benefits Schedule (MBS) items which will also effect the Medicare rebates for all allied health services.
The upcoming changes, effective from July 1st, 2025 will introduce new MBS items under the GP Chronic Condition Management Plan (GPCCMP) which replace the existing GP Management Plans (GPMP) and Team Care Arrangements (TCA) and review items.
This is now known as a GP Chronic Condition Management (CCM) Plan and is the structured approach to managing one or more chronic medical conditions.
It was previously called rebates under the Chronic Disease Management (CDM) Plan and before that it was formally known as Enhanced Primary Care (EPC) or is often referred to as a GP Care Plan.
These changes will help simplify care for both patients, General Practitioners and Allied Health Professionals. All these changes shouldn't effect our patients at all and it will be your GP that will be implementing the changes when preparing your referral to our clinic.
Already a patient with a GPMP or TCA?
If you are a current patient and already seeing one of our exercise physiologists in our allied health gym that has an existing GPMP or TCA prior to July 1st will be able to continue accessing care under that plan or arrangement. That means that any current chronic disease management plan won’t have to be interrupted or redone due to the new CCMP framework.
Patients who are not currently on a GPMP or TCA on July 1st will be able to make use of the new CCMP item numbers, while patients on a GPMP or TCA prior to July 1st can continue to access care as they normally would for the following two years.
